File a Complaint
The Boards and Commission Division takes every legitimate complaint against our licensees seriously and investigates to determine if any professional or ethical standards have been violated. Depending on the severity of the violation, the Division can suspend or permanently revoke a license.
To file a complaint please complete the complaint form,
providing the individual board or commission as much information as possible about your complaint. List any other people who might have information or knowledge about this situation, and include the addresses and phone numbers for each of them. You must have your signature notarized by a notary public. Please type and print your complaint form or write legibly so the staff could understand the nature of the complaint.
