Renewal information

Your license expires January 30. Remember, you cannot practice with an expired license.

To renew your license, submit the completed and signed appropriate renewal form to the Board, including payment by check, money order, or credit card, payable to the Board, for the amount indicated on the Fees page.

All renewals must be postmarked no later than January 30 or there will be a $50 late fee added. The $50 late fee will be added to renewals postmarked during the grace period of January 31 thru March 31.

Licenses not renewed by the end of the grace period, March 31, must reapply for licensure and meet all applicable requirements.

Incomplete renewal forms will be returned. To avoid a late penalty, be sure to include your list of CEU's and the appropriate renewal fee with your completed, signed renewal form.

Clinical Fellow Year (CFY)

A Clinical Fellow license expires one year after the initial issue date. Renewal notices are sent one month prior to the expiration date. The renewal must be received in the board office postmarked no later than the expiration date, along with a check, money order, or credit card payment of $50, payable to the Board. Documentation of successful completion of the National Exam must be submitted with your renewal form.

If the renewal is not postmarked on or before the expiration date, there is a 30 day grace period to renew; however, you must pay a late fee of $50.

Temporary Hearing Aid Permit

The Temporary Hearing Aid Dispensing Permit expires one year after the initial licensure date. Renewals are not allowed.
