Complaint Procedures
The statutory authority granted to the Regulation and Licensing Department’s Respiratory Care Advisory Board is limited. Disciplinary measures may only be imposed against licensees who have been found to have violated the Respiratory Care Act or 16.23 NMAC, the regulations governing respiratory care practitioners.
Complete the complaint form, providing the Board office with as much information as possible regarding the complaint against the respiratory care practitioner in question.
- List any witnesses who might provide information related to your complaint, including the address and phone number of each. A written affidavit from any witness is advisable if at all possible.
- Sign the form in the presence of a notary public.
- Complete the authorization for disclosure of health records, if applicable.
The complaint form must be typed or printed legibly or it will be returned to you.
