Complaint Procedures
Be aware that the Physical Therapy Board’s jurisdiction is limited. It cannot obtain refunds of money paid to licensees, nor necessarily obtain the desired service for you. It can only impose disciplinary measures against a licensee found to have violated the licensing act or rules.
Download and complete the complaint form.
- Include the name of the occupational therapist or occupational therapist assistant you are filing a complaint against.
- List any witnesses and include the address and phone number of each.
- Sign the form on pages three and four in the presence of a notary public.
- Complete the Authorization for Disclosure of Medical Records.
The complaint form must be typed or printed so it is legible or it will be returned to you.
