Download and complete the complaint form.
Provide the Board office with as much information as possible about the complaint. List any other people who might have information or knowledge about this situation and include the address and phone number of each.
Forms must be typed or printed so they are legible or they will be returned to you.
Sign the form in the presence of a notary public and have it notarized.
